All mentors working with students are required to have criminal history background checks prior to interacting with any students.  These checks should be completed and submitted to the ITEST Grant Dropbox.  Submit the files to initest@lehigh.edu using the “Drop-off” feature on the Dropbox website.

Please save the report as a PDF file and submit along with your payment receipt.

Please save the report as a PDF file and submit along with your payment receipt.

Federal Criminal History Record Information (CHRI) – FBI Report
Please save a copy of your registration and payment information as a PDF and submit.  You must provide your Registration ID.
